New Patients:

• Please call (952) 241-4050, to schedule your first appointment.

• After you have called and before your scheduled appointment, you MUST read all the forms attached below:

  • Notice of Privacy Practices

  • Telemedicine Policies and Procedures

• And you MUST fill out ALL of these forms below. This step will take you about 30-60 minutes to complete.

  • New Patient Packet

  • Telemedicine Consent Form

  • Patient Contact Information

  • Patient Rights and Responsibilities

  • Medicare Authorization

  • Treatment Agreement

• (Optional) If you would like to send your previous medical history to your new provider please fill out the form below:

  • Release of Information

• If you would prefer to print out and mail your updated forms, please use the attachments below:

  • New Patient Packet
